Referrals

Three steps:

1) Enquire—tell us about the person and goals.


2) Assessment—we meet, co-produce an initial plan and a costed proposal.


3) Start & review—agree a date, match staff and review early to ensure a smooth transition.

We support adults (18–65) with learning disabilities, autism, mental health conditions, dementia and other associated complex needs.


We’ll confirm eligibility, discuss any reasonable adjustments, and outline any assessments required. We’ll also keep you updated with realistic timescales and next steps.
